RESUMO

PURPOSE: Implement a constructivist approach in thoracic drainage training in surgical ex vivo pig models, to compare the acquisition of homogeneous surgical skills between medical students. METHODS: Experimental study, prospective, transversal, analytical, controlled, three steps. Selection, training, evaluation. INCLUSION CRITERIA: a) students without training in thoracic drainage; b) without exposure to constructivist methodology. 2) EXCLUSION CRITERIA: a) students developed surgical skills; b) a history of allergy. (N = 312). Two groups participated in the study: A and B. Lecture equal for both groups. Differentiated teaching: group A, descriptive and informative method; group B, learning method based on problems. A surgical ex vivo pig model for training the chest drain was created. Were applied pre and post-test, test goal-discursive and OSATS scale. RESULTS: Theoretical averages: Group A = 9.5 ± 0.5; Group B = 8.8 ± 1.1 (p = 0.006). Medium Practices: Group A = 22.8 ± 1.8; Group B = 23.0 ± 2.8 (p <0.001). CONCLUSION: Through the constructivist methodology implemented in the thoracic drainage training in surgical ex vivo pig models, has proven the acquisition of surgical skills homogeneous compared among medical students.

OBJECTIVE: to describe and evaluate the acceptance of a low-cost chest tube insertion porcine model in a medical education project in the southwest of Paraná, Brazil. METHODS: we developed a low-cost and low technology porcine model for teaching chest tube insertion and used it in a teaching project. Medical trainees - students and residents - received theoretical instructions about the procedure and performed thoracic drainage in this porcine model. After performing the procedure, the participants filled a feedback questionnaire about the proposed experimental model. This study presents the model and analyzes the questionnaire responses. RESULTS: seventy-nine medical trainees used and evaluated the model. The anatomical correlation between the porcine model and human anatomy was considered high and averaged 8.1±1.0 among trainees. All study participants approved the low-cost porcine model for chest tube insertion. CONCLUSION: the presented low-cost porcine model for chest tube insertion training was feasible and had good acceptability among trainees. This model has potential use as a teaching tool in medical education.

Objetivo: descrever e avaliar a aceitação do modelo porcino de baixo custo para drenagem torácica em projeto de ensino médico no oeste do Paraná, Brasil. Métodos: um modelo suíno de baixa tecnologia e baixo custo foi desenvolvido em projeto de ensino de drenagem torácica. Alunos de Medicina e médicos residentes receberam instruções teóricas sobre o procedimento e realizaram a drenagem torácica no modelo porcino. Após realizarem o procedimento, os participantes responderam a um questionário sobre o modelo experimental proposto. Esse estudo apresenta o modelo e analisa as respostas ao questionário. Resultados: setenta e nove participantes usaram e avaliaram o modelo. A correlação anatômica entre o modelo porcino e a anatomia humana foi considerada alta com média de 8,1+1,0. Todos os participantes aprovaram o modelo porcino de baixo custo para o ensino de drenagem torácica. Conclusão: o modelo porcino de baixo custo para drenagem torácica apresentado neste projeto de ensino foi facilmente montado e teve boa aceitação local entre os participantes. Esse modelo tem potencial para ser usado como ferramenta de ensino na educação médica.

OBJECTIVE: To evaluate the acceptability of an educational project using A porcine model of airway for teaching surgical cricothyroidotomy to medical students and medical residents at a university hospital in southern Brazil. METHODS: we developed a teaching project using a porcine model for training in surgical cricothyroidotomy. Medical students and residents received lectures about this surgical technique and then held practical training with the model. After the procedure, all participants filled out a form about the importance of training in airway handling and the model used. RESULTS: There were 63 participants. The overall quality of the porcine model was estimated at 8.8, while the anatomical correlation between the model and the human anatomy received a mean score of 8.5. The model was unanimously approved and considered useful in teaching the procedure. CONCLUSION: the training of surgical cricothyroidotomy with a porcine model showed good acceptance among medical students and residents of this institution.

OBJETIVO: Avaliar a aceitabilidade de um projeto de ensino utilizando modelo porcino de vias aéreas no ensino da cricotiroidotomia cirúrgica para estudantes de Medicina e médicos residentes em um hospital universitário no sul do Brasil. MÉTODOS: Foi desenvolvido um projeto de ensino usando modelo porcino para treinamento em cricotiroidotomia cirúrgica. Estudantes de Medicina e residentes receberam aula teórica sobre esta técnica cirúrgica e, em seguida, realizaram no modelo o treinamento prático. Após o procedimento, todos os participantes preencheram um formulário acerca da importância do treinamento em manuseio de vias aéreas e do modelo utilizado. RESULTADOS: Houve 63 participantes. A qualidade geral do modelo porcino foi estimada em 8,8, enquanto a correlação anatômica entre o modelo e a anatomia humana recebeu o escore médio de 8,5 entre os treinandos. O modelo foi unanimemente aprovado e considerado útil no ensino do procedimento. CONCLUSÃO: O treinamento de cricotiroidotomia cirúrgica em modelo porcino apresentou boa aceitação entre os estudantes de Medicina e os residentes desta Instituição.

INTRODUCTION: Rene De Garengeot, a French surgeon, was the first to describe the presence of the appendix inside a femoral hernia sac in 1731. It is a rare entity that has fewer than 100 cases reported in literature. PRESENTATION OF CASE: An 86 years-old male patient, comes to Emergency Department complaining of painful bulging in the right inguinal region, associated with local inflammatory signs. He was initially diagnosed as incarcerated femoral hernia and underwent emergency open surgery. Inguinotomy was performed and after hernia sac dissection it was possible to observe the presence of the appendix incarcerated in its interior, without clinical signs of appendicitis. Surgeons performed appendectomy and inguinal repair of the femoral hernia with placement of a polypropylene mesh. DISCUSSION: De Garengeot hernia is a rare entity that requires early treatment in order to avoid possible complications. When facing a patient with incarcerated hernia emergency surgery must be indicated even if it is not possible to determine the contents of the hernia. CONCLUSION: This paper presents a case report of a De Garengeot hernia patient who presented a good evolution after surgery.

RESUMO

A associação de pneumoperitônio, dor abdominal e íleo paralítico no período pós-operatório relaciona-se, na maioria dos casos, à perfuração de vísceras ocas. Os autores apresentam um caso de pós-operatório de cesariana por DPPNI em que a paciente evoluiu com importante distensão e dor abdominal. Realizada radiografia de tórax e abdome evidenciou-se importante dilatação das alças de intestino grosso e delgado e pneumoperitônio, foi submetida a laparotomia exploradora com a hipótese diagnóstica de perfuração intestinal. Na cirurgia, no entanto, não foi encontrada lesão gastrintestinal; foi confirmado o diagnóstico de pneumoperitônio, identificando-se hemoperitônio e hematoma subaponeurótico infectados (E.coli). A evolução pós-operatória foi satisfatória, mantendo-se antibioticoterapia (ceftriaxona + metronidazol) por 4 dias. A paciente teve alta no 7§ dia pós-laparotomia. Depois de revisão da literatura pertinente, os autores concluem que o presente caso de pneumoperitônio relacionou-se possivelmente à infecção por bactéria produtora de gás em paciente com quadro clínico de íleo paralítico.
